How to make a referral

Please contact our Clinical Admin team to make a referral. They will help to make sure the referral form is completed with all necessary information to progress it forward.

What happens next?

A referral for support is made

We will call parents/ carers directly for a phone assessment

Screening against criteria

Care plan written for early support or long-term support

Delivery of 10-weeks of early-support or long-term support for ‘complex needs’.

Please note, a plan of care will be made during the first three sessions along with a decision around delivery.
